Patient Rights and Responsibilities

At Superior Family Health Team we believe that to create a circle of care, health care providers, patients and families must work together as partners to achieve quality health care.

As a patient of Superior Family Health Team, you have a right to:

  • Receive considerate, compassionate and respectful health care services
  • Receive care without discrimination as to race, age, religion, disability, gender, sexual orientation or national origin
  • Receive care from skilled health care providers
  • Expect that all information about your care will be kept private according to the law and that all communications with health professionals will be confidential
  • Be able to access information from your health record according to the law
  • Be fully informed about one’s medical condition, available treatment options, and treatment decisions
  • Designate a person to exercise rights on the patient’s behalf if the patient is not able to do so because of a physical or mental incapacity
  • Know if clinical students may be helping in your care, under supervision, and that you have the right to refuse if you do not wish to have students participate in your care

As a patient of Superior Family Health Team, you have a responsibility to:

  • Provide health professionals with full and accurate information relating to your health and the public health services you have received
  • Keeping all appointments and notifying providers in advance if this is not possible
  • Participate in decisions about your care and treatment
  • Accept responsibility for the decisions you make about your treatment
  • Cooperate with health professionals and follow their advice or indicate when you have not done so
  • Be polite and respect other patients and members of the health care team
  • Comply with prescribed treatments and ask questions and tell your health care provider if you do not understand
  • Give your health care team the name of the person who will represent you in the event that you cannot make decisions for yourself
